Home Health Agencies CARE IN THE HOME Provide a wide range of health and social services delivered at home to persons recovering from an illness or injury, or persons who are disabled and/or chronically ill. They provide "skilled services" such as nursing, social services, and therapeutic treatments (physical, speech, occupational therapy). They also provide non-skilled services like help with bathing, dressing and eating. Medical equipment such as wheelchairs, walkers and oxygen may also be provided.
Hospice Programs TERMINAL PATIENTS Provides palliative care consisting of medical, social, and support services for a period of six months to persons who are terminally ill.
Dementia / Alzheimer's Care Also known as memory care or dementia facility. This type of facility specializes in the treatment and care of people with Alzheimer's and Dementia as they have special needs. The setting is similar to Assisted Living communities. It also provides personal services like help with eating, bathing, toileting, grooming, dressing-up, etc. Social services and programs tailored to provide patients with as much mental and memory stimulation as possible are also available. These facilities have 24-hour support, higher level of security to protect wanderers, and structured programs to meet the needs of people with dementia.
Nursing Facility LONG TERM CARE An establishment that provides food, shelter, and nursing care for four or more persons who are unrelated to the owner and provides minor treatment under the direction and supervision of a physician, or other services that meet some need beyond the basic provisions of food, shelter or laundry.
Adult Day Care Facilities Provides counseling, recreation or food, or any combination of these services on a regular basis, but not overnight, to four or more elderly persons or persons with disabilities, not related to the owner.
Independent Living Also referred to as Retirement Communities. This is ideal for healthy and active seniors who want to live independently as they want flexibility with their day-to-day activities. Seniors who choose this option typically do not need medical assistance. This lifestyle is suited for seniors who are interested to participate in educational, cultural and other social activities for entertainment. These facilities usually offer a wide range of optional personal services like: cleaning, laundry, transportation, group meals, fitness programs, etc.
Rehabilitation Centers Rehabilitation centers offer many different services, including physical, speech, and occupational therapies. Many attendees have disabilities or ongoing medical problems, while others are recovering from an accident or a medical incident such as a stroke. Rehabilitation centers are often affiliated with another health care organization, such as a hospital or nursing home, although they can be separate, freestanding facilities. Private Duty CNA Many private duty nurses are self-employed, or work as contractors. Most nurses who provide private duty care are working one-on-one with individual clients. Sometimes such care is provided in the client's home, or an institution, such as a hospital, nursing home or other such facility. Assisted Living Provides services to eligible adults who require access to care on a 24-hour basis but do not require daily nursing intervention. Services include, but are not limited to, personal care, home management, escort, 24-hour supervision, social and recreational activities, transportation, food, and room.
Mental Health Centers Care for mentally impaired or emotionally disturbed individuals. Will work under a team that may include psychiatrists, psychologists, psychiatric nurses, social workers, and therapists. In addition to helping patients to dress, bathe, groom themselves, and eat, aides socialize with them and lead them in educational and recreational activities. Aides may play games such as cards with the patients, watch television with them, or participate in group activities, such as sports or field trips. Observe patients and report any physical or behavioral signs that might be important for the professional staff to know. Accompany patients to and from examinations and treatment.
Residential Care Home Offers 24 hour personal care and supervision in a residential home - like setting. Semi-private and private rooms are available. Some offer private rooms with private bathrooms. Services include meals, personal care and laundry, housekeeping, social activities and medication management. Recommended for the older person who is dependent on someone else for his or her care and safety.
Respite Care Respite care is short term care that helps a family, or caregiver take a break from the daily routine and stress. It can be provided in the client's home or in a variety of out of home settings. Respite care is an essential part of the overall support that families may need to keep their loved one at home.
Sitter / Companion Service This type of program is for clients needing non medical care. Care is typically provided in a clients home, but can be in a hospital, or different settings. CNAs can work hourly or live in shifts. CNAs assist with housekeeping, bathing, grooming, meals, transportation, monitoring medications, errands, incontinent care, transferring and mobility assistance. Some CNAs are hired for quality of life reasons, such as for companionship, or taking the client to social events Basic nursing skills (ie blood pressure, temperature or vital signs) are not provided in sitter service care
